Readmissions Rates

Preventable rehospitalization could account for as much as $12 billion (source: Jencks, SF et al. “Rehospitalizations among patients in the Medicare fee-for-service program.” N Engl J Med 2009;360:1418-28) in health care expenses every year. Clearly, it can only benefit our health care system as a whole – not to mention the experience of the individual patient – to develop standards that reduce readmissions. Our hospitals are leveraging data collected from across our system to identify patients at high risk for readmission and provide them with the information and post-hospital treatment they need to stay healthy – and home – longer.

Reducing Length of Hospital Stays

At Partners, we understand that reducing the length of hospital stays both lowers costs and improves the patient experience. Using data collected throughout the Partners system, we are working to deliver the right care at the right time in the right place for more of our patients – for example, by directing them towards primary or urgent care instead of emergency departments, when appropriate. That means lower costs for private and government payers without sacrificing quality or patient outcomes.
