Patient Safety: Serious Reportable Events:

Patient Protection Events

The occurrence of Patient Protection Events is extremely rare. All Partners hospitals have implemented strategies to prevent these events which include well constructed policies and procedures for verifying the identity of newborns and their parents as well as for assessing patients for their risk of harming themselves while in our care.

While there have been no instances of these events in 2008, the Partners hospitals are required to report the following:

  • An infant is discharged to the wrong person
  • Patient death or serious disability associated with patient elopement (disappearance)
  • Patient suicide or attempted suicide resulting in serious disability , while being cared for in a healthcare facility
