Partners physicians keep track of the percentage of patients with coronary artery disease, peripheral vascular disease or diabetes to make sure these patients have had their LDL, or "bad" cholesterol, checked in the past year. Making sure that a patient’s level of LDL cholesterol is properly managed decreases the risk of future blood vessel problems and complications.
For this measure, our practices perform this important screening 94% of the time, outperforming the state average and matching the national 90th percentile.
Partners physicians use the reminders and scheduling systems that work best to make sure patients have their “bad” cholesterol levels checked regularly. Systems making this easier are the electronic medical record and other electronic systems that can identify patients at risk or who have not been tested, and then help the physicians send a reminder to these patients or test them at their next appointment.
