What are we measuring and why?

Asthma is one of the most common chronic illnesses in children. Asthma is the reason for a significant number of hospitalizations and visits to the emergency room, as well as missed school days. This measure tracks the percentage of patients aged 5 to 11 who have persistent asthma symptoms and have received a prescription for medication to manage their asthma.

How are we doing and how do we compare to  best practice?

The overall score for Partners community practices is 98%, which is slightly above the Massachusetts state average of 97%. Our practices' scores range from 92% to 100% on this measure.

What are we doing to improve? 

Our physicians use reminders and reporting systems to ensure that patients with persistent asthma receive appropriate medical management. which can help identify these patients and remind physicians to prescribe medications to prevent and treat their symptoms. The electronic medical record and tools like it track a patient’s visits and medication use – whether they were hospital stays or trips to the emergency room.  This information is securely stored and made available for authorized caregivers to view, so that patients receive better coordinated care.
