Prevention and Chronic Condition Management:

Well Visits for Adolescents Ages 12 – 21

What are we measuring and why?

Adolescent patients’ well-visits are an important time to check their growth and development, as well as an opportunity to discuss ways to avoid risky behaviors.

This measure looks at the percent of adolescent patients between the ages of 12 and 21 who had at least one well visit in the past year.

How are we doing and how do we compare to best practice?

Seventy-five percent of our adolescent patients ages 12 – 21 have had a well visit in the past year, which is above both the Massachusetts state average and the National 90th percentile.

What are we doing to improve?

Pediatric practices use their practice management systems and electronic health records to track well child care and to make sure that patients are scheduling and keeping appointments for well child visits.
