Some patients have unique care needs that require specialized programs to ensure that they remain as healthy as possible. For example, patients with heart failure may need frequent hospitalization, which makes heart failure one of the most challenging and costly conditions to manage. Partners HealthCare has created outpatient programs that help patients like these thrive at home, keeping them healthy enough to avoid hospitalization while reducing cost to the nation’s health care system.
We work to improve care for other special groups of patients as well. Partners HealthCare Connection is a telephonic health-coaching program for Medicaid and financially eligible patients with a primary care doctor at BWH, MGH or NSMC. Connection began enrolling patients in August 2004. To date 7,600 Medicaid patients have been coached by health professionals. Health Coaches make sure their patients are filling their prescriptions, taking the medications, and keeping appointments. To read more about this program, please click here.
