Patient Safety Measures

We all know that, despite the advancements in modern medicine, medical errors and harmful events still occur. Partners HealthCare is committed to reducing our patients’ risk of experiencing a harmful event or medical error. Using technology coupled with recommended safe practices and coordinated teamwork, our clinicians are working to ensure that care at Partners HealthCare facilities is as safe as possible.

Safe Medication Administration	See Hospital Detail		We are on track to achieve the Partners internal goal of having 100% of patients, staff, and medications barcoded by 2009, and the eMAR software implemented by 2010.
Ensuring Safer Transitions in Care	82% October - December 2011	75% Partners-wide Goal	There is no nationally recognized benchmark for this measure. Partners has set our interim goal at 75%, and we are aiming for even higher levels of adherence. On October 1, 2009 we changed this measure to include new elements that are important in safe transitions in care and "retired" other elements that were successfully implemented. We expect that our scores will get better over time as we address areas in need of improvement.
Transitions in Care: Timeliness of Discharge Documentation	83% Data Period: January - March 2012	75% Partners-wide Goal	There is no nationally recognized benchmark for this measure. We have set our internal goal to 75%, and we are aiming for even higher levels of achievement.
Central Line Associated Bloodstream Infections	See Hospital Detail Data from January - June 2009. For this metric, a lower value is better.	For this metric, a lower value is better. Comparison Group is the Centers for Disease Control and Prevention, National Healthcare Safety Network Report, data summary for 2007, issued November 2008. Am J Infect Control 2008; 36:609-26.	We measure the rate of Central Line Associated Bloodstream Infections by ICU type for each of our hospitals. See detail for each hospital's performance.
Preventing Patient Falls in the Hospital	See Hospital Detail		At this time, we measure the number of patient falls by hospital unit, compared to the same units in the other hospitals in MA. See detail for each hospital's performance.
