We all know that, despite the advancements in modern medicine, medical errors and harmful events still occur. Partners HealthCare is committed to reducing our patients’ risk of experiencing a harmful event or medical error. Using technology coupled with recommended safe practices and coordinated teamwork, our clinicians are working to ensure that care at Partners HealthCare facilities is as safe as possible.

There is no nationally recognized benchmark for this measure. Partners has set our interim goal at 75%, and we are aiming for even higher levels of adherence. On October 1, 2009 we changed this measure to include new elements that are important in safe transitions in care and "retired" other elements that were successfully implemented. We expect that our scores will get better over time as we address areas in need of improvement.
